Community Health Needs Assessment Overview

The Windsor Health Department has received funding for a Mental Health Promotion and Suicide Prevention Grant. As part of this, the Department is conducting a Community Health Needs Assessment (CHNA) to identify community health priorities, with a focus on mental health and suicide prevention. The CHNA is being developed with support from the Windsor Mental Health Alliance, which includes community leaders working to improve mental health locally. The process involves analyzing health data and gathering community input on health experiences and perceptions. As a valued member of the Windsor community, your feedback is essential. Please complete the survey below!

After completing the survey, add your comments to the social map to share additional input and highlight areas of the community that help you stay healthy or make it harder for you to be healthy. Please complete the survey before adding your comments to the map to help us make Windsor a healthier place to live, work, and play!

Additional information and resources related to improving mental health and preventing suicide in Windsor are available on the Town of Windsor's Minds Matter page, linked here.

The CHNA is supported by the Preventive Health and Health Services Block Grant (PHHSBG) funded by the Centers for Disease Control and Prevention.
